Accreditation Certificate Order Form

The CAATE understands what an achievement accreditation is for your Athletic Training Program and is pleased to begin offering a complimentary accreditation plaque and certificate to programs as they receive their accreditation award. Each program will receive one plaque and certificate suitable for display at the institution. Programs will receive an updated certificate for subsequent accreditation cycles – comprehensive review only. When a program transitions degrees, the certificate does not change. Programs have the option to order additional plaques and/or certificates at any time.

Please fill in the information below that will be printed on your certificate, and provide a current mailing address to ensure delivery of your items to the correct location. At the bottom of the form you will have the opportunity to place orders for additional items, however, purchase is not required. Please skip this section if you are not ordering additional items. Orders are processed by the CAATE Office at the beginning of each month and typically take 6-8 weeks to arrive at your institution.
